Enrollment Consent, Media Release & Liability Waiver

1. About This Agreement

By signing the enrollment, you (the Parent/Guardian) confirm the consents below for the named Player. This agreement is between you and the Academy that operates the program; SkillTrackr provides the software used to record it.

Consent is specific to one child and one enrollment. Enrolling another child, or a new program for the same child, requires a fresh signature so that the permissions and information are confirmed each time.

2. Parent/Guardian Authority & Acknowledgement

You confirm that you are the parent or legal guardian of the Player and have full authority to enrol them and to give the consents in this agreement. You confirm that the information you provided — including the Player's date of birth, medical conditions, allergies, and emergency contacts — is accurate and current, and you agree to update it if it changes.

3. Photo & Media Consent

You consent to the Academy photographing and recording video of the Player during training, matches, and events. You understand these images and recordings may be used for coaching analysis, internal Academy communications, the Player's progress records, and — where you have separately enabled the social-media/communications option — Academy newsletters, websites, and social-media channels.

You may decline or withdraw the social-media use at any time without affecting the Player's participation. Withdrawing consent does not require the removal of materials already published before the withdrawal where removal is not reasonably practicable.

4. Medical Treatment Authorisation

In the event of injury or illness during an Academy activity where you cannot be reached in time, you authorise Academy staff to administer or arrange first aid and to seek emergency medical treatment for the Player, including transport to a medical facility.

You agree to be responsible for the costs of any such medical treatment and to disclose any condition, allergy, or medication that staff should know about to keep the Player safe. This authorisation does not make the Academy or SkillTrackr responsible for providing medical care.

5. Assumption of Risk & Liability Waiver

You acknowledge that participation in sports training and competition involves inherent risks, including the risk of serious physical injury, and that these risks cannot be eliminated. You voluntarily accept these risks on behalf of the Player.

To the fullest extent permitted by law, you release and agree not to hold liable the Academy, its staff and volunteers, and SkillTrackr for any injury, loss, or damage arising from the Player's participation, except to the extent caused by gross negligence or wilful misconduct. Nothing in this waiver excludes any liability that cannot lawfully be excluded.

6. Player Code of Conduct

You agree that the Player will follow the Academy's reasonable instructions, safety rules, and code of conduct, will treat coaches, staff, and other participants with respect, and will use facilities and equipment responsibly. The Academy may suspend participation for serious or repeated breaches.

7. Withdrawal of Consent

You may withdraw an optional consent (such as social-media use or video/AI analysis) at any time through your account settings or by contacting the Academy. Some core consents — such as the assumption of risk and emergency medical authorisation — are necessary for safe participation, and withdrawing them may mean the Player can no longer take part.

8. Electronic Signature

By typing your full legal name and submitting the enrollment, you provide your electronic signature. You agree that your electronic signature has the same legal effect as a handwritten signature and confirms your agreement to this Enrollment Consent, Media Release & Liability Waiver, the Terms of Service, and the Privacy Policy.
